Work it out! is a chronic disease self management program that aims close the life expectancy gap between Indigenous and non-Indigenous Australians through addressing chronic disease. The group program adopts a holistic view of Indigenous health and utilises an inter-professional health partnership approach to aid in chronic disease self management in South-Eastern Queensland. The 12 week program includes: weekly 45 minute educational sessions delivered by allied health professionals weekly individually tailored one hour group exercise sessions one-on-one meetings with health professionals to aid in self management strategies which are unique to the individual chronic disease. The program also has a research component which monitors both qualitative and quantitative health indicators including blood pressure, blood glucose and social and emotional wellbeing. Since it's inception in 2011, the program has shown statistically significant improvements for these indicators.
